Obverse: The Purple Heart Medal, bearing the image of George Washington beneath his personal coat of arms, is featured on the obverse, along with five stars which represent the U.S. military's five branches. Inscribed "COMBAT WOUNDED & KILLED IN ACTION," "LIBERTY," "1932," "2022" and "IN GOD WE TRUST."

Reverse: A female nurse wearing a World War I style helmet is depicted on the reverse, as she carefully bandages a wounded soldier on a stretcher. The inscriptions "UNITED STATES of AMERICA" and "E PLURIBUS UNUM" and "ONE DOLLAR" sweep above the image. Obverse and reverse designs of the coin were created by U.S. Mint Artistic Infusion Program artist Heidi Wastweet.
