While the U.S. Mint is very careful to produce all coins within strict quality guidelines, the Mint isn't perfect. The results of the Mint's mistakes are available here in the Mint Errors category. Different errors that you will encounter are:
Off-Center: A coin is not struck centered on the blank, resulting in an oblong coin where part of the design is missing, and part of the blank remains unstruck
Double-Struck: A coin is struck not once, but twice. Usually the coin is struck on-center the first time, and off-center the second time. Additional strikes are possible, but much rarer.
Saddle Strike: A multiple strike coin where each strike is off-center.
Broadstrike: The coin is struck outside of a collar, allowing it to spread out and have a diameter larger than a typical coin.
Brockage: Part or all of a coin is struck with a mirror image of the design due to another coin remaining stuck in the die during the striking.
Recently, the U.S. Mint has gotten much stricter on quality control, and errors are far rarer. When a batch of coins is found to have errors, the entire batch is destroyed. The process that destroys these coins is known as waffle-cancelling. The only waffle cancelled coins to be released have been Missouri quarters and, in much smaller numbers, proof Sacagawea dollars.
